Welcome to South Middle School Nurses' Page

We are committed to promoting the health and well-being of all our students. Our roles include providing direct health services, managing chronic conditions, health education, and advocating for a safe and healthy school environment.

• Students may not carry and/or self-administer medications. All Over-the-Counter and Prescription medications require a physician’s order and parent consent. This needs to be renewed yearly. Forms can be found using this link. Nurse Forms            
• If there is a life-saving medication that needs to remain with your student, please contact the nurse for appropriate paperwork.
• Whether your student needs assistance with illness, injury, medication management, or health-related questions, we are here to help.  Feel free to contact us with any questions you may have.
